Licensed Nursing Assistant - Full Time/Various Shifts

Under the supervision of the LPN Charge Nurse or Staff Nurse, provides basic resident care and assists in the maintenance of the unit environment.

Follows facility policies and procedures and the standards of the Nurse Practice Act of New Hampshire.

Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.



 Staff RN/LPN - Various Shifts

Under the direction of the Nurse Manager or RN Supervisor, responsible for performing clinical and administrative duties on assigned floor.

Supports the Nurse Manager or RN Supervisor in maintaining standards of resident care and maintaining policies and protocols to meet OBRA regulations for long term care.

Duties require knowledge of nursing equivalent to completion of an applicable Nursing Program.

Must be licensed by the State of New Hampshire Board of Nursing.
Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
Must attend mandatory in-services and retain registration with the State of New Hampshire.
 


MNA

GENERAL SUMMARY:

Under the direction of the charge nurse, assists with routine prescribed medication administration and documentation in accordance with established nursing standards, and the policies, procedures, and practices of this facility.  Administers PRN medications upon the instruction of the delegating licensed nurse. 

ESSENTIAL JOB FUNCTIONS:

·        Receive report at the beginning of shift with nurses. 
·
       
Prepare, administer and chart oral, topical, suppository, nasal, ocular, and otc medications, as prescribed by the physician, and as directed by the delegating licensed nurse through shift assignment and Medication Administration Record (MAR).
·
       
Document administration on the MAR.
·
       
Administer PRN medications as prescribed by the physician, only as directed by the delegating licensed nurse. Delegating licensed nurse must be available to monitor the effects on the resident.  Documents same on MAR. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

·        Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.
·
       
Must have successfully completed a state approved Medication Nursing Assistant Program.

GENERAL SUMMARY: Under the supervision of the Food Services Director, responsible for providing assistance with the preparation, set-up, and serving of scheduled meals.

 

ESSENTIAL JOB FUNCTIONS:
·       
Obtains food and supplies as instructed.
·        Performs cleaning duties in the kitchen and dining room; gathers dirty
 dishes from the resident floors; scrapes dishes, loads dishwashing machine; and puts away clean dishes.
  Ensures a sanitized and clean environment.

·       
Works on the tray line; cuts up breakfast tickets.
·       
Assists in serving meals and loading food.
·      
Assists in setting up dining room, including setting up juices, butters, desserts, and breakfast and lunch specials for the following day.

GENERAL SUMMARY:

Under the direction of the Director of Occupational Therapy, responsible for evaluating, treating, and monitoring residents’ abilities to safely consume nourishment and effectively communicate in their daily lives.  All responsibilities are in accordance with governmental, state, and facility practices and standards.

ESSENTIAL JOB FUNCTIONS:* 

·        Organizes and conducts resident’s speech-language program to facilitate rehabilitation or habilitation of a resident. 

·        Consults with other members of the team in order to coordinate with other therapeutic/rehabilitative activities. 

·        Selects rehabilitative and therapeutic treatment based on evaluations; determines a treatment program appropriate for the resident.   

·        Works closely with Dietary and Occupational Therapy staff on program recommendations. 

 

Duties require knowledge of speech language pathology equivalent to completion of a Master’s degree and three to five years of related experience, or equivalent combination of education and experience.  New Hampshire Allied Health Professional’s license to practice as a SLP required.  Must attend mandatory in-services and retain license with the State of New Hampshire.

Responsible for directing and supervising all operations of the assisted living apartments in accordance with county, state, and federal rules, regulations and laws.   

ESSENTIAL JOB FUNCTIONS:

Coordinates, oversees, and provides administrative guidance for operations of the assisted living apartments on a twenty-four hour basis.

Responsible for pre-admission assessments of prospective residents of the assisted living apartments, as well as ongoing monitoring of medical condition of residents.

Prepares and presents the annual operating budget; monitors funds and tracks line item expenditures in accordance with established policies and procedures.

Prepares required reports, records, and statistics in accordance with local, state, and federal regulations.

Promotes favorable public relations for the facility and arranges for marketing development activities, including distributing brochures and setting up tours.

Responds to questions, inquiries, and complaints related to facility operations. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

  • Duties require knowledge of assisted living facility management equivalent to completion of an Associate’s degree along with advanced professional courses and three to five years of related experience, or equivalent combination of education and experience. 

  • Must be a Registered Nurse and hold current registration with the New Hampshire Board of Nurse Examiners.

  • Must have a genuine interest in geriatrics and assisted living facility administration.

  • Must possess a thorough knowledge of and background in health care administration, including sanitation, safety, business management, personnel administration, and governmental licensure requirements.

  • Must be abreast of new developments and current trends in health care.

  • Must have the ability to display patience, tact, and understanding when working with staff, residents, family members, and the public.

  • Ability to recognize assisted living facility-wide priorities and work cooperatively to support their accomplishment.

  • Financial management and strong organizational skills.

  • Ability to manage multiple priorities.

  • Ability to act as a role model for employee morale and good employee relations.

  • Ability to market programs and ideas; to communicate effectively both verbally and in writing; to establish positive public relations, and to interact effectively with a wide variety of people.

SUPERVISORY RESPONSIBILITY:

Directly supervises the Assisted Living employees.

GENERAL SUMMARY:

Under the supervision of the Housekeeping Supervisor, responsible for performing a wide range of cleaning duties in all areas of the Nursing Home, using specialized housekeeping equipment and chemicals in order to maintain a clean, safe, and aseptic environment for residents, staff, and the public..

ESSENTIAL JOB FUNCTIONS:

·        Follows cleaning schedule as outlined for Housekeeping Department to include discharged/isolations cleaning procedures.

·        Cleans all assigned building areas throughout the Nursing Home in a clean, neat, and sanitary manner and in accordance with established housekeeping procedures.

·        Operates various types of cleaning equipment, both mechanical and electrical, in order to remove soils, stains, and other debris from assigned areas.

·        Cleans all resident rooms and corridors on a daily basis; disinfects when necessary; polishes where required.

The Cheshire County Sheriff’s Office

Candidates should be available for shifts on days, nights and weekends.  On call personnel train for eventual part time/ full time employment as positions open. Requires operating complex communications system and the ability to communicate professionally and effectively with the public.  Excellent multi-tasking, typing, and listening skills are required.  Must be able to demonstrate tact and calmness in stressful situations. High School graduate with associate degree or equivalent experience with some police and/or public safety/emergency dispatching preferred.
